PURSUANT to the Securities and Exchange Commission (SEC) En Banc Resolution dated February 27 2020, the deadline for the submission of the Mandatory Disclosure Form (MDF) has been moved to March 31 this year.
The submission of MDF is required of all non-stock/non-profit organizations (NPOs) as mandated under SEC Memorandum Circular No. 25 series of 2019.
All SEC Registered Non-Stock Corporations may fill out the form online by accessing the same through the link http://forms.gle/dxkUMJJtcekE2NUk9.
After filling out and submitting the form online, a printable copy of the accomplished form will be automatically sent to the email address indicated in the form.
Corporations may access this link to the Declaration/Verification Page through http://www.sec.gov.ph/2019mandatorydisclosuredeclarationverificationpage/.The Declaration/Verification Page shall be attached to the duly filled out MDF.
Moreover, the MDF that was filled and submitted online and emailed back shall be printed with the attached Declaration/Verification Page duly signed and notarized and submit one original copy to the Enforcement and Investment Protection Department (EIPD), Secretariat Bldg., PICC Complex, Roxas Blvd. in Pasay City or to the nearest SEC Extension Office on or before March 31 2028.
Only the printed copy of the accomplished MDF form filled out and submitted online with the Declaration/Verification Page will be accepted.
Failure to submit the MDF is a ground for the revocation of one's Certificate of Recognition.
